Support at Home Package Management Noosa & Buderim – Your Support at Home Care Collective
Tracey brings over 16 years of experience in aged care, having worked with private and not-for-profit organisations across Home Care Packages, CHSP, NDIS and transitional care services. With more than 10 years of experience in case management, she has also held leadership roles managing large teams supporting clients in the community. While she has enjoyed these roles, her true passion lies in working directly with older Australians and their families to provide personalised, compassionate care.
Tracey takes a person-centred approach, building lasting relationships and taking the time to understand what matters most to each client. She values open communication and believes everyone deserves to feel heard, respected and involved in decisions about their care. Outside of work, Tracey enjoys cooking, spending time with family and friends, and relaxing by the water.

The Noosa and Buderim region is located on Queensland’s Sunshine Coast, stretching from Noosa Heads and Tewantin in the north through Peregian Beach and Coolum Beach to Maroochydore and Buderim in the south. The region includes suburbs and towns such as Noosaville, Sunshine Beach, Cooroy, Peregian Springs, Bli Bli, Mountain Creek and Sippy Downs.
Local Guardians supports older Australians throughout the region with personalised care management, helping them make the most of their Support at Home funding, access suitable services and maintain their independence at home.
Older Australians living in Noosa and Buderim, including Noosa Heads, Tewantin, Noosaville, Peregian Beach, Coolum Beach, Maroochydore and surrounding suburbs, can apply for an aged care assessment through My Aged Care.
An aged care assessment helps determine eligibility for government-funded services, including Support at Home, residential aged care, respite care and transition care.
Assessments are free and available to people aged 65 and over, Aboriginal and Torres Strait Islander people aged 50 and over, and people aged 50 and over who are homeless or at risk of homelessness, provided they meet the care needs requirements.
During the assessment, a qualified assessor will discuss your health, daily activities and care needs to recommend suitable support. Family members or carers can participate with your consent.
Several organisations provide aged care assessments throughout Noosa, Buderim and the Sunshine Coast under the Single Assessment System, which replaced the previous ACAT and Regional Assessment Service arrangements.
Assessment organisations servicing the Sunshine Coast aged care planning region include Aspire4Life, Centacare, Queensland Department of Health, Footprints and Suncare.
